When your account is created, your representative will discuss the fees associated with creating your custom forms. Should you require any forms to be updated, or altered, the fees chart below will help determine the cost.

- Add or alter single field – Includes conditional logic $10
- Add a single field – Includes conditional logic, HTML and Image $15
- Add, or alter a notification destination $5
- Alter a completed form confirmation $5
Altering and/or creating new fields is time consuming and requires updating other areas of the form to maintain functionality. We have done our best to keep fees reasonable considering the amount of time required to create, or modify forms.
For your convenience, an invoice will be created for any fees accrued.
